Our Support Options

Designed for individuals who already have a higher level of independence and do not need frequent support or direction throughout the week.
Includes:
One individualized visit each week
Home or community-based support
Focus on specific personal goals
Help with routines, organization, responsibilities, or everyday skills as needed
Community outings when they support the individual’s goals
Personalized visuals, schedules, or checklists as needed

Designed for individuals who are working toward independence but still need regular hands-on support, guidance, and help putting those skills into practice.
Includes:
Two individualized visits each week
Home and community-based support
Transportation during scheduled support when appropriate
Regular community outings and opportunities to practice independence
Help finding and connecting with classes, programs, activities, volunteer opportunities, and community resources
Support attending and becoming comfortable with community activities
Help with routines, responsibilities, social skills, and everyday independence
Personalized tools and parent/caregiver communication

Designed for individuals who need consistent support throughout the week and hands-on help across several areas of everyday life.
Includes:
Three individualized visits each week
Consistent home and community-based support
Transportation during scheduled support when appropriate
Hands-on support with routines, responsibilities, daily living skills, organization, and independence
Regular community outings and activities
Help finding, accessing, and maintaining programs, social opportunities, volunteer experiences, or employment opportunities
Weekly planning and follow-through
Personalized tools, parent/caregiver communication, and coordination with appropriate supports
Priority scheduling

Not every individual or family needs ongoing monthly support. Limited à la carte services may be available based on current availability, with pricing determined by the type and level of support requested. Consistent weekly scheduling cannot be guaranteed with à la carte services.
